Guidelines for Membership Type
  General Membership.General Membership in the Association is open to anyone who is actively
involved in or responsible for the clinical aspect of discharge planning/continuity of care process,
who is currently employed in the health care field, i.e., Acute Care and Rehabilitation Facilities,
Sub-Acute Care and Skilled Nursing Facilities, Workers Compensation/Independent Case
Managers, or Insurance/IPA/Medical Group based Case Managers; and who also agree with the
philosophy and objectives of the Association.  General Members shall have full voting status and may
serve as Board or Committee Members.  Actively involved shall include, but not be limited to,
directly related employment of no less than an average of 20 hours per week.  A General Member in
good standing is defined as one who has attended six (6) out of the last ten (10) meetings.

Associate Membership.  Associate membership in the Association is available to any representative
of a firm, partnership, corporation, or other entity which has a mutual interest in continuity of care
through the delivery of services and products, or any person who wishes to contribute to the
on-going efforts of the Association, and who agrees with the purpose and philosophy of the
Association (hereafter "Associate Member"), but who does not qualify as a General Member.  
Associate Members may serve on the Board with no voting rights; they shall be eligible to attend
monthly meetings, and they may serve on committees or subcommittee.
